1) The controller of my personal data is Infonet Projekt Spółka Akcyjna with registered office at 94 Bystrzańska St., 43-300 Bielsko-Biała, District Court in Bielsko-Biała, 8th Economic Department KRS No. 0000323192, NIP 553-20-87-889, fully paid share capital: PLN 1,201,000,00 (hereinafter: Infonet Projekt SA).
2) The basis for the processing of my data is the consent I have expressed voluntarily, consciously, specifically and unambiguously.
3) My personal data is processed in accordance with the principles specified under the Privacy Policy and in accordance with the Resolution of the Parliament and of the Council (EU) 2016/679 of 27 April 2016 on the protection of natural persons with regard to the processing of personal data and on the free movement of such data and repealing directive 95/46/WE (hereinafter: “GDPR”)
4) My personal data is collected only for the needs of realisation for my benefit of the purposes indicated in the consent given by myself.
5) The data referred to under the consent will not be made available to other entities except from those required by the execution of the purpose for which the consent has been given.
6) In case it is needed to provide the personal data to the states outside of the European Economic Area (“third states”), before the data of the user is handed over, Infonet Projekt S.A. will make sure that the recipient of the data will provide appropriate protection of the rights of the user and the appropriate level of protection of the personal data – that is meeting the requirements resulting from the laws of the European Union.
7) The data will be stored for the period of execution of the purposes specified under the consent.
8) The provision of the data covered by this statement is voluntary.
9) I am entitled to demand from the controller the access to my personal data, its correction, removal or restriction of processing and to raise an objection against processing or transfer of the data.
10) I am entitled to complain at a competent supervising institution with regard to the personal data protection in case my data is processed in a manner that is not in compliance with the effective regulations.
11) I am entitled to withdraw the expressed consent at any time, however, without prejudice to the compliance with the processing right that has been executed on the basis of the consent prior to its withdrawal.
